Home School Enrollment Forms
Welcome to the 2017-18 school year new & returning home school families!
The homeschool packet for the 2017-18 school year is ready. Please remember that the information you provide us is mandatory by the state. It would be greatly appreciated if you could return the forms to the district office by September 15, 2017 so that we have time to compile the information that the state requires by its due date in October. If complete information is not received, the district must report the Home School as “Not Compliant” in our report to the State of Minnesota. Click on the underlined packet that is appropriate for your home school this year, download it, print it, fill it in, and scan/email back to me, or put in the U.S. mail.
- New Princeton Home School Families – Complete the Full Home School Packet.
- Returning Home School Families - Complete the Letter of Intent to Continue to Provide Instruction.
- All Home School Families – Complete the Student Report for Aids to Non Public Students form for textbook, health services, and/or guidance counseling reimbursement (found in either packet or linked here separately).
Fill out the Immunization form if you are new or have a new child you have are beginning to homeschool this year, or you have been homeschooling and your child has new immunizations since the last form you sent in.
* In order to be eligible for reimbursement, you are required to complete and return the Student Report for Aids to Nonpublic Students form by September 15, 2017, if possible, so that we can compile the forms in order to send them to the state office by their due date. Original receipts must be received by May 1, 2018 and dated during the fiscal year which started July 1, 2018. Checks will be sent out no later than June 30, 2018.
