Every fall families need to verify their student and family information, along with filling out some annual forms. To get started, log in to Family Access and locate the "Back to School Forms" button on the left-side menu. For each student in your family, you will:
- Verify contact information for your student
- Complete Application for Educational Benefits (optional)
- Complete Residence Questionnaire
- Sign the Responsible Use Agreement
- Sign the Handbook Form
- Sign the Field Trip Permission Form (grades K-8)
Note: each step must be complete before you can submit the forms. Once you submit your forms, your child's schedule can be unlocked to view in Family Access.
