Transportation Request

Effective July 1, 2019 families can access the Transportation Request Form in Skyward Family Access. Please read through this page for instructions to help you complete this form. When you are ready, please click Skyward at the top of this page to log into Skyward Family Access.  Allow 3-5 days for changes to be processed.

STEP 1: Login to Skyward Family Access

• Click on Online Forms on the left side menu.
• Locate the Transportation Request Form and click to fill out form.

STEP 2: If this is your first bus form this school year:

• Click next on the right side to begin.
• In the pop up box, click yes to begin form.
• Fill out required information on form & electronically sign the bottom.
• Scroll to top of form and click “save”.
• Click “next step” on the right side of form.
• Click “Submit Transportation Request Form” button at bottom of screen.

OR, If you are changing your bus form:

• Click on the “Mark as not completed and make changes” link to unlock form.
• Click on “Step 1 Transportation Request Form”
• Click the “Edit Step 1” button at bottom of screen.
• Fill out required information on form & electronically sign the bottom.
• Scroll to top of form and click “save”.
• Click “next step” on the right side of form.
• Click “Submit Transportation Request Form” button at bottom of screen.
